Assistant Manager- Disease Management

The Job Involves:

• Leading in disease management

• Build and maintain a data base for those members on chronic disease treatment

• Implement disease management protocols

• Review utilization of benefits

• Ensure and monitor adherence to treatment protocols of members on chronic medication

• Carry out regular updates of any changes in treatment plans by Practitioner to ensure continuous smooth access to treatment and prevent duplication of treatment plans

• Liaise with providers on patient compliance to treatment • Recommend to management the emerging disease management protocols

Person Specification: 
1. Degree in General Nursing or any relevant degree
2. Post graduate qualification in either Intensive Care Nursing, Midwifery or Public Health will be an added advantage
3. At least five years’ experience in healthcare with knowledge of healthcare finance
4. Management of chronic diseases and design of disease management protocols an added advantage
5. This is a managerial position which requires a self-motivated individual with strong analytical skills.
6. Good communication skills
7. Good knowledge of office procedures and practices
